How to Order an Installation, Removal, or Change in Telephone Service

All requests for installation, removal, or change in telephone service must be approved by the appropriate department head.

Go to:

Request for Microcall access

Please send an email to telecom@umassd.edu with the following information:

  1. Name of Department
  2. Name of person who will be given access
  3. Building
  4. Room number

Request for an Extension

Please send an email to telecom@umassd.edu with the following information:

  1. name of persons telephone will be assigned to
  2. department name
  3. building and room number where instrument will be installed
  4. jack number (the number on the label of the phone outlet)
  5. class of service (see class-of-service)

Telephone switch (Inter-office)

Please send an email to telecom@umassd.edu with the following information:

  1. name of person telephone will be assigned to
  2. department name
  3. building and room number where instrument will be installed
  4. jack number (the number on the label of the phone outlet)
  5. CLASS OF RESTRICTION (see CLASS OF RESTRICTION)
  6. speed type for billing purposes
  7. reason for the move
  8. name and extension of the second phone
  9. jack number (the number on the label of the phone outlet)

Request for a change in COR (Class of Restriction)

Please send an email to telecom@umassd.edu with the following information:

  1. name of person requesting change in class of service
  2. department name
  3. extension number
  4. new class of service requested
  5. reason for change in class of service

Activating a Telephone

Please send an email to telecom@umassd.edu with the following information:

  1. name of person telephone will be assigned to
  2. department name
  3. building and room number where instrument will be installed
  4. jack number (the number on the label of the phone outlet)
  5. class of service (see class-of-service)
  6. speed type for billing purposes
  7. reason for the installation ( phone service or computer hook-up)

Activating a New Fax Machine

Please send an email to telecom@umassd.edu with the following information:

  1. existing fax Number
  2. department name
  3. building and room number where instrument will be installed
  4. jack number (the number on the label of the outlet)
  5. reason for the installation

Request for a SUB Extension

Please send an email to telecom@umassd.edu with the following information:

  1. name of persons telephone will be assigned to
  2. department name
  3. building and room number where instrument will be installed
  4. jack number (the number on the label of the phone outlet)
  5. class of service (see class-of-service)

Telephone Move

Please send an email to telecom@umassd.edu with the following information:

  1. name of person requesting move
  2. department name
  3. present location of telephone - building and room number
  4. new location for telephone - building and room number
  5. jack number of both locations
  6. extension number
  7. reason for move

New Telephone Request

Please send an email to telecom@umassd.edu with the following information:

  1. name of person telephone is assigned to
  2. department name
  3. building and room where new phone will be located
  4. extension number
  5. speed type for billing purposes.
  6. reason for request (IE; new employee, upgrade, broken phone, or stolen phone.)

Installation of a Telephone

Please send an email to telecom@umassd.edu with the following information:

  1. name of person telephone will be assigned to
  2. department name
  3. building and room number where instrument will be installed
  4. jack number (the number on the label of the phone outlet)
  5. class of service (see class-of-service)
  6. speed type for billing purposes
  7. reason for the installation ( phone service or computer hook-up)

Checking Phone line

Please send an email to telecom@umassd.edu with the following information:

  1. name of person assigned to
  2. department name
  3. building and room number
  4. jack number (the number on the label of the phone outlet)
  5. class of service (see class-of-service)
  6. speed type for billing purposes
  7. reason for the inspection
  8. extension number

To reset voice mail:

  1. dial into audix 508 910 5001 (on campus just 5001)
  2. at the prompt, enter your phone extension and password (the default password is 2244)
  3. to reset your header name, enter 5, follow the system prompts, and record your name. To save the new name, enter 5 again.
  4. to change the greeting, enter 3 and follow the prompts
