Forms

• The student hiring form (Federal Work-Study and non work-study) is available from the Student Employment Office.  It is not available in a downloadable format.

             The following forms may be downloaded:

• W-4 - Withholding Allowance Certificate submitted to the Student Employment Office.
  
  
• I-9 - Employment Eligibility Verification submitted to the Student Employment Office. This form requires certain original forms of documentation to be presented at the time it is filled out.  Acceptable documents are listed on the back of the form.
  
  
• Direct Deposit Form is turned into the Payroll Office, Admin Building, Room 202.
  
  
• OBRA Participation Agreement is required for all students working on-campus in the summer who are enrolled in less than 6 credit hours.

• Student Position Description Form for Community Service Learning Program is only for off-campus supervisors hiring Federal Work-Study students.
  
  
• Work'n Serve Job Fair Registration Form  is for all on-campus employers wishing to participate in the September 5, 2007 job fair.
  
  
• Student Employment Evaluation Form  is for supervisors to use when conducting Student Employee Employment Reviews.
