Non-Unit Dental Plan Open Enrollment October 17 – November 18, 2016
On behalf of the Trustees of the Non-Unit Employee Health and Welfare Fund, we invite you to participate in the non-unit dental plan. The Non-Unit MetLife Dental Plan continues to be among the best dental plans offered to public employees in the Commonwealth, providing:
· 100% coverage for preventive services
· 80% coverage for basic restorative services
· 50% coverage for major restorative services
· a low annual deductible of $25 per person, with a $75 family limit
· an annual maximum benefit of $1,500 for each covered individual
· coverage for dependents, including orthodontia, up to age 26
By enrolling, employees are also able to take advantage of the MetLife vision discount program.
If you are a Non-Unit eligible employee and desire a change in your dental coverage please call our Benefits person Joanne Costa at x 8083 and she can assist you in your change or sign up. As in the past, employees who want their dental coverage status to remain the same will do nothing.
Employees will need to return the completed forms by November 18th in order to ensure your ability to establish the necessary December payroll deductions. New coverage and coverage changes will be effective on January 1, 2017.

You do not have to do anything to continue your coverage. If you would like to make a coverage change or drop coverage please make the change on the enrollment form and return to Joanne Costa in Human Resources by the deadline.
