F-1 Job Offer - Students with an F-1 Visa need their potential on-campus employer to fill this out in order to obtain a Social Security Number.
Transfer Out - International students transferring from UMD to another school must fill this form out.
Family Visit - Students whose parents or family would like to visit them in America need a letter for a temporary visa. Please fill in your information and return to the International Student Center so we may place on letterhead.
Graduation Letter - Students who are graduating commonly request this letter for family members to be allowed a visa to enter the country. Please fill in your information and return to the International Student Center so we may place on letterhead.
Change of Address - Students who change address at any time in the U.S. must inform the International Student Center so we may comply with SEVIS.
Declaration of Finances - Please be sure to complete and submit a Declaration of Finances.
Program Extension Request - Students who are applying for program extension, reinstatement, returning from a leave of absence, or are adding a dependent must use this form.
Reduced Course Load Approval Form - Fill this out and turn it in for reduced course load.
