Common Forms for International Students, Faculty, and Staff at UMass Dartmouth

• Pre-Arrival Checklist - Students who may find the Pre-Arrival page a bit confusing can take a look at our comprehensive pre-arrival checklist to see that they have what they need prior to departure. 
• F-1 Job Offer - Students with an F-1 Visa need their potential on-campus employer to fill this out in order to obtain a Social Security Number.
• Undergraduate Transfer (In) Verification - Undergraduate International Students transferring into UMD must complete this form and the International Advisor from their previous institution must provide the relevant SEVIS information.
• Graduate Transfer (In) Verification - Graduate International Students transferring into UMD must complete this form and the International Advisor from their previous institution must provide the relevant SEVIS information.
• Transfer Out - International students transferring from UMD to another school must fill this form out.   
• Family Visit - Students whose parents or family would like to visit them in America need a letter for a temporary visa. Please fill in your information and return to the International Student Center so we may place on letterhead. 
• Graduation Letter - Students who are graduating commonly request this letter for family members to be allowed a visa to enter the country. Please fill in your information and return to the International Student Center so we may place on letterhead. 
• Change of Address - Students who change address at any time in the U.S. must inform the International Student Center so we may comply with SEVIS.
• Program Extension Request - Please be sure to complete and submit a Declaration of Finances.   
• Declaration of Finances - Students who are applying for program extension, reinstatement, returning from a leave of absence, or are adding a dependent must use this form.
• Reduced Course Load (RCL) Approval - Students must submit this form by the end of add/drop if eligible for an RCL. Please read the conditions under which your faculty person may recommend a reduced course load.
