Clinical Requirements:

All RN students must keep their files current every year by submitting:

1. Verification of a current TB result and Varicella documentation Immunization Form (obtain and return to Dean's Office, in the College of Nursing)
2. Verification of MA RN License (photocopy license and return to Dean's Office in the College of Nursing)
3. Verification of current CPR Certification (photocopy CPR card and return to Dean's Office in the College of Nursing)

In addition if you have not already done so please complete and return:

5. Sign and return the enclosed Bloodborne Pathogen Sheet to the Dean's Office in the College of Nursing, if you have not already done so.
6. Sign and complete the enclosed CORI Form to the Dean's Office in the College of Nursing, also, a copy of your driver's license is needed.
