Artistic Achievement Award Fund
REQUIREMENTS AND PROCEDURES
1. Provide notarized verification of:
(a) at least one great-grandparent who is/was a native of the Cape Verde Islands
or
(b) verification of two African-American parents, including birth certificates, photos, and a notarized statement.
2. Request that official university, college or conservatory transcripts be sent to this Fund by the issuing institution and received by August 18, 2005. You must be a full-time student in good standing, currently enrolled and entering your third year at an accredited institution.
3. Complete and submit the official Application form for the "Ethel Ramos Harris Artistic Achievement Award" as well as a biographical statement.
4. Submit three letters of recommendation, one from each of two (2) recent piano instructors and one of character reference.
5. Perform three (3) advanced classical piano selections from memory via an acoustical piano as listed. State your name and selections prior to performing. One selection must be a Prelude and Fugue from Johann Sebastian Bach's "Well-Tempered Clavier". One selection must be the "Scherzo, Opus 31 in Bb minor" by Frederic Chopin. One selection may be the applicant's choice. Combined performance time for all three selections must not exceed thirty (30) minutes.
Total performance must be on VHS videotape or DVD (with full view of piano and performer) and sent by August 18, 2005 to the following address:

6. All documents and materials submitted in the application process must be received by August 18, 2005. They must be true and correct or the application will be null and void. All submitted materials become the property of the Fund and will not be returned.
