Below are links you can download in prepration for competition as a varsity athlete at UMass Dartmouth.

The letter contains all of the information you need to know about what is required before you can play varsity sports at UMass Dartmouth. You can download the two forms, fill them out and return them to the UMass Dartmouth Training Room.

Note: You will need Microsoft Word, Excel, or a program capable of reading Excel and Word documents to open these forms properly. If you cannot open the files, or encounter problems with opening and printing the forms, please use another program that understands Excel and Word documents.

  All student athletes must fill out and submit the Insurance Verification Form and the Shared responsibility for sport safety form. First-year and transfer student athletes must fill out the Comprehensive Medical History Form, while returning student athletes must fill out the Updated Medical History Form.
