Health Form

Congratulations on your acceptance to UMass Dartmouth.

In order to securely, accurately, and efficiently collect your medical information, Health Services has provided you with access to Vivature, a secure web services application in which you will enter your medical and family history, as well as the mandatory immunization information required to attend the university. This information must be entered by January 15, 2010.

Note: This procedure is for all new incoming students, beginning spring 2010. Current students who have not yet completed their health forms will need to pick them up at the Health Office.

To access your pre-created Vivature account, please visit:

https://www.vivature.com

and log in with the default user name and password that has been assigned to you. Your default account information is as follows:

User name: UMass Dartmouth email username (example: jdoe@umassd.edu)

Password: Student ID (example: 11111111)

Changing your password

After you log into Vivature, you must change your default password. Prior to inputting any information into Vivature, please select My Account from the upper right area of the page, and scroll down to the Change User Name/Change Password section to modify your information.

Entering medical information

After you change your password, please visit the My Records section of Vivature (found on the upper left portion of the page) and input information into the forms listed under My Records.

Health and wellness tools

In addition to providing forms to enter your medical and immunization information, Vivature has a "Health Library" section, which contains a collection of health and wellness tools available to all students to research symptoms and access news articles on a variety of topics including health, dietary, emotional, sexuality and other wellness topics.

Due dates

These requirements are due:

  • Fall acceptance: June 1 (all students) or August 15 (late acceptances)
  • Spring acceptance: January 15
  • Or 30 days after acceptance

Failure to comply will adversely affect your admission and result in a hold on your registration. Residential students will not be able to stay in the residence halls until the health information is entered in Vivature.

Physicals

The Health Services office does not require physicals for students. However, students participating in Intercollegiate Athletics will need a physical. The form is available from the Athletic Department. More information
