Health Services

Immunization Requirements

The Commonwealth of Massachusetts requires that all full-time students submit documentation of :

  1. Two doses of live measles vaccine given at least 1 month apart beginning after 12 months of age; initial vaccines must be after 1967.
  2. Two doses of mumps vaccine given at least 1 month apart beginning after 12 months of age; initial vaccines must be after 1967.
  3. Two doses of rubella vaccine given at least 1 month apart beginning after 12 months of age; initial vaccines must be after 1967.
  4. A booster dose of Tetanus/Diphtheria within last 10 years.
  5. Three doses of Hepatitis B vaccine.
  6. Two doses of Varicella (chicken pox) vaccine when age 13 or older, given after 12 months of age and at least 4 weeks between doses.Children age 12 months to 12 years only receive a single dose.

The health form

You will enter your medical and family history, as well as the mandatory immunization information required to attend the university on the online health form.

These immunizations are required by June 1st (all students) or August 15th (late acceptances) for Fall acceptance, and January 15th for Spring acceptance, or 30 days after acceptance. Failure to comply will adversely affect your admission, result in a hold on your registration, and residential students will not be allowed into the residence halls. Please mail in your records by the due dates.

The meningitis vaccine is not typically available in the Health office. However, once a year in late October/early November, the meningitis vaccine is available through the flu clinic for $125. Otherwise, please check with your physician, the local public health department, travel clinics, or your local hospital (occupational health).

Residential students

All new residential students must either receive the meningitis vaccine or sign a waiver form (PDF available) before they can reside on campus and attend school. All students living on campus must have these documents in the Health Office by June 1st (all students) or August 15th (late acceptances) for Fall acceptance, and January 15th for Spring acceptance, or 30 days after acceptance.

All Residential students (full-time, part-time, continuing education, matriculated and non-matriculated) MUST comply with all of the above immunization requirements.

*Note: Non-matriculated, continuing education students who live in the residence halls must comply with the immunization requirement; however, they are not eligible to use Health Services since they are not charged a health fee.

Most students have a record of having these injections at their physician's office or their high school. If records cannot be found, Health Services can re-immunize for $15. These immunizations are only available during the academic semester, and when we hold special immunization clinics. Please put money on your UMass Pass card (use the "UMDollars" account), and call Health Services for an appointment.

Nursing and Med Lab Science majors

Nursing majors and Med Lab Science majors must have all of the above requirements for both full-time and part-time students. Nursing students participating in a clinical rotation must also have a current PPD. Nursing students (excluding RN-BSN) must have proof of varicella (chicken pox) immunity.

Failure to comply

Effective now, failure to comply will result in residential students losing their housing to persons on the waiting list, and all students will have classes dropped from his or her academic schedule, just prior to the start of classes. 

Flu clinics

Every fall in late October or early November, we offer flu vaccines through a vendor for $30. The day and times are advertised through university emails.
