If you have been the target of a bias incident (or have witnessed a bias incident) you may complete a Bias or Hate Anonymous Reporting Form.

Your confidentiality is very important. Every effort will be made to protect your privacy. Anonymity cannot be guaranteed in cases involving the safety of the community, those warranting criminal action, and some violations of university policies.  The form does allow for anonymous reporting. However, the University’s ability to respond to incidents of bias and to prevent recurrence may be limited with anonymous complaints.

We encourage you to report this incident to all other appropriate offices (e.g. campus police, Housing and Residential Life, Office of Diversity, Equity and Inclusion) in addition to filing this report with the Bias Response Team.
