Request to take a leave of absence, have previously earned credits be applied to your degree, or make a transition in your program.
- Leave of Absence form: For students requesting a temporary leave of absence from the university with the intent to return.
- Graduate Student Advanced Standing Request Form: For doctoral students requesting advanced standing credits to be applied toward their doctoral degree, based on a previously earned master's degree from another institution.
- Degree Program Transition Request Form: For students requesting master’s-level transitions or transitions from doctoral to master’s programs. Examples include switching formats within the same master’s program (e.g., MBA in-person to MBA online), changing to a different master’s, or declaring a master’s “en route” to a doctoral program to which you are already admitted.
