Student Forms
Student Electronic Forms
Students seeking to submit any of the following electronic forms should log into COIN for Students and select My eForms from the left-hand navigation. Then, select the appropriate form from the dropdown menu.
- Change of Program (Undergraduates only): For undergraduate students requesting to change, add, or drop a major, minor, or concentration.
- Course Overload: For students requesting to enroll in credits beyond the standard course load limit during a given semester or session.
- Change Expected Graduation Term: For students to update their expected graduation term.
- FERPA Release Authorization: For students to grant or update an individual's access to their student records.
- FERPA Directory Restriction: For students to restrict or release the Directory Information contained in their student record.
- Incomplete Grade Request: For students requesting an incomplete grade in a course due to exceptional circumstances.
- Student Leave of Absence/Withdrawal: For students requesting a temporary leave of absence from the university with the intent to return or a voluntary withdrawal if they do not plan to continue their enrollment.
