Forms
The following forms can be downloaded here, and should be filed with the Graduate Program Director:
- Intention of Track form (PDF): The Physics MS has three tracks: MS with Thesis, MS with Research Project, and MS with Comprehensive Exam. File this form with the Graduate Program Director to state which track you are pursuing.
- Thesis Committee form (PDF): If pursuing the MS with Thesis, this form should be filed at least 4 months (and preferably 6 to 8 months) before the student is expected to finish the program. The completed form should be submitted to the Graduate Program Director together with a thesis prospectus that outlines the research project.
- Thesis Defense form (PDF): This form is filled out by the Thesis committee upon completion of the Oral Defense of the Thesis.
- UMassD Accelerated Program Pre-Application Form Physics (PDF): BS/MS intent declaration form. Give the completed form with a brief statement to the Physics Department Administrative Assistant. The same statement may be used when you submit the official application. No reference letters/GRE etc. needed.
